There are two options this for submission of your CPC requests:

Complete an electronic application: This allows you to complete and submit your request directly through the website. A pdf of your application will be generated and emailed to you for your records.
Complete a fillable pdf: Same process as last year; download and complete the pdf, save it, then email your application and any additional information.
